Please note: We cannot process your application without proof of eligibility.
The procedure is as follows:
- Fill out the form below.
- Provide your IBAN for any refunds.
- Attach a scan of your proof to the form.
- We will review your eligibility and change your membership accordingly if necessary.
- If you are eligible for a refund for an annual fee or deposit that you have already paid, we will issue it.
- Please be sure to use the email address you used to register with MOQO.
If you have any questions, please always include the ticket number in the subject line, which will be provided in the email you receive after submitting the form.
